Your First Visit
We want your first visit to be smooth and comfortable. Please plan to arrive about 10 minutes early so we can complete your registration and answer any insurance questions. Bring a photo ID, your insurance card, and a list of current medications.
New Patient Forms
You can save time at your first appointment by downloading, printing, and completing the following forms before you arrive. If you are unable to print them, we are happy to provide copies at the office.

Insurance & Payment
Our office works with most major PPO dental insurance plans. We also accept cash, checks, and major credit cards. Payment is due at the time of service, and our front desk will provide a cost estimate before any treatment.
What to Expect
Your first appointment includes a comprehensive exam, digital X-rays as needed, and a conversation about your goals and concerns. We will give you a clear, honest treatment plan and never recommend care you do not need.
